- Please include as much information as possible so I can provide the best tattoo for you!

- For some pieces I may request an in person consultation and I will let you know upon receiving your booking form. 

- All details are important ! Ex: If you have specific flowers you would like or colour accents , include them in the "Detail" section on the booking form. 

- If you are looking to get script included in a tattoo please include a font from dafont.com or I will design one for you. 

- Include any reference photos in the upload file section.

- Upon completion of this form, you will receive a reply to confirm potential booking dates.

- Once we have agreed on a date and time, I will send a deposit agreement form, which must be completed in order to secure the appointment.
